Event Management

The undersigned parent/legal as a parent, legal representative, or both, on behalf of the athlete by name hereby, knowingly and willfully, releases and discharges Atlanta United Football Club LLC and other related entities from any and all liability, claims, demands, actions, causes of action, costs and expenses of any nature whatsoever, which the undersigned or the athlete may now have or hereafter acquire.

This release extends to and includes all injuries, damages, illnesses, or property damage occurring directly or indirectly from participation in any activities. The undersigned acknowledges that participation involves risks including serious injury or death and accepts all such risks.

By checking this box, I understand that as well I verify every conceivable risk cannot be listed herein and I hereby assume all risks whether or not included in the forms and other waivers provided.
